Outpatient Laboratories

To help promote patient safety and social distancing, RUMC campus labs now require an appointment. To make a lab appointment, watch this video or use the following steps: 

  1. Download the My Rush app or go to MyChart. Login with your MyChart credentials. If you don't have a MyChart account, you can create one for free. 
  2. Navigate to Appointments > Future and Past Appointments > Schedule an Appointment.
  3. Select Lab Visit. 
  4. Answer the questions, select your appointment location and your preferred appointment time. 

There are three full-service outpatient laboratories at Rush. 
Please note: Patients requiring a 1 hour glucose tolerance test must present to the lab 1 hour and 30 minutes before the lab closes to ensure proper preparation and processing involved for this test.

Lab registration

Please register at outpatient registration before receiving any laboratory service. To register, bring your insurance information and photo ID.

You also need a laboratory requisition completed by your physician. The following information is required on the laboratory requisition:

  • Full name, sex and date of birth
  • Medical record number (if known)
  • Physician’s full name
  • Physician’s group practice (if applicable)
  • Tests requested
  • ICD-9 code or diagnosis

Lab billing

The outpatient labs accept the following forms of payment: cash, personal check, MasterCard, Visa, and American Express.

Payment and billing varies upon your medical coverage. This is what you can expect at the outpatient labs at Rush if you have the following types of coverage:

Blue Cross/Blue Shield of Illinois →
  • Present your insurance card as proof of insurance.
  • Rush will bill Blue Cross/Blue Shield.
  • If you have out-of-state Blue Cross/Blue Shield, you may not qualify for coverage. In that case, lab personnel will ask you to pay (with cash or credit card) at the time of service.
Other commercial insurance →
  • Present a completed insurance claim form.
  • Rush will bill the insurance company directly.
  • If you do not have claim forms, lab personnel will ask you to pay (with cash or credit card) at the time of service.
Medicare and Medicaid →
  • Present a current Medicare/Medicaid  card.
  • Rush bills Medicare and Medicaid directly.
RUSH-PRUDENTIAL health plan and RPSLMC contracted providers →
  • Present an enrollment card and/or authorized form.
  • If you have certain HMOs or providers not contracted with RPSLMC, lab personnel will ask you to pay (with cash or credit card) at the time of service.
  • If you have questions about contracted providers, please call (312) 942-5841.
Uninsured/noncontracted providers →
  • Payment (with cash or credit card) is due at the time of service.
Contracted services →
  • If your insurance requires that you go to a specific laboratory for testing, please present a requisition form (completed by your physician’s office) from the required laboratory.
  • The outpatient laboratories at Rush will send the specimens collected to the specified laboratory.

If your insurance requires you to use a specific laboratory, we will send specimens to the following laboratories:

  • Athena Diagnostics
  • NMR LabsPrometheus Laboratories Inc.
  • Quest Diagnostics

In some cases, you or your insurance company may receive a bill from these laboratories.
