Social Work and Community Health

Health care can seem confusing and overwhelming. The Department of Social Work and Community Health addresses barriers to health and wellness by providing a wide variety of innovative programs and services that engage and support patients, caregivers and community members.

Our programs address the following questions and concerns you might have:

I am a caregiver seeking support.
I was just discharged from the hospital.
I just got a new diagnosis or medication and I feel overwhelmed.
I want to learn more about resources for improving my health, preventing disease and living my best life.
I am experiencing symptoms of depression or anxiety and would like to learn about services.

You can connect with many of our services through your Rush care provider or discharge planner. You can learn more by calling the Social Work and Community Health 800 line at (800) 757-0202. The 800 line is gateway to different programs with different eligibility criteria.
