What Is Rush Connect Primary Care?
Rush Connect Primary Care is an innovative primary care clinic dedicated to helping you reach your health goals. As a patient, you’ll have access to a team of providers including a physician, advanced practice provider, social worker, medical assistant, registered nurse and pharmacist. You can stay in touch with your providers without leaving the house through MyChart messages and virtual appointments. Most appointments are virtual, but your provider may ask you to attend some visits in-person.
How does Rush Connect Primary Care Work?
A MyChart account is the easiest way to access Rush Connect Primary Care. The MyChart platform allows you to contact your care team any time with questions or concerns. They’ll message you with important health care reminders and check in on ongoing issues.
You can also contact your Rush Connect Primary Care providers by phone and visit the clinic in-person.
You can use MyChart to schedule appointments with your provider. The Rush Connect Primary Care Team is available for virtual visits four days a week, and for in-person appointments one day a week.

Personalized Care for Chronic Conditions
If you have a chronic condition, you may need more support from your health care team. If you have two or more chronic conditions, our Advance Primary Care Management, or APCM, program can help you stay on top of your care. The APCM program will check in with you outside of your regular appointments. They’ll reach out regularly to answer any questions you may have. They’ll also stay in contact with any specialist providers to make sure everyone is on the same page about your health care plan. The cost of the program depends on your insurance.

Most visits for the clinic are virtual. In some cases, you may need to visit the clinic in-person if the care you need cannot be delivered virtually.
Rush Connect Primary Care accepts many forms of insurance, including Medicare. Please check with your insurer to make sure the location or provider you plan to visit is in-network before your appointment, as accepted insurance plans can change without notice.
