Rush Connect Primary Care is an innovative clinic dedicated to making care more accessible.
Virtual-first: Most Rush Connect Primary Care visits take place via telehealth, meaning you can get the care you need without leaving home. In-person appointments are available at our Chicago office when you need them.
Compassionate care: Rush Connect Primary Care maximizes the amount of time you have with your care team. Your providers will give you clear information, and they’ll create a care plan tailored to your social needs and health goals.
Support when you need it: If you have multiple chronic conditions, Rush Connect Primary Care providers will check in with you, helping you stay on top of your health needs and answering any questions you have.
Parking for Rush Connect Primary Care is located at the main parking garage. The garage is across the street from the main entrance to Rush University Medical Center at 1620 W. Harrison St.
Rush Connect Primary Care is in the Professional Building. The building is connected to the parking garage via the Paulina Street bridge.
If you are being dropped off, enter through the main entrance to the Professional Building at 1725 W. Harrison St. Valet parking is also available at this entrance.

You can schedule an appointment with Rush Connect providers Michael Cui, MD, Tabassum Faruqi, MD, and Donna Gaona, NP, online. Existing Rush patients can also schedule through MyChart.

Rush Connect Primary Care providers can treat a wide range of non-emergency conditions. The clinic has a special focus on managing chronic conditions, including diabetes, high blood pressure and kidney disease.
Before your appointment, you’ll receive pre-visit questions from your care team. Be sure to complete these before your visit so you have as much time with your provider as possible. Your provider will discuss your medical history and current concerns with you during your first visit.
