Connect to Care and COVID-19 Testing through Connect Chicago

Connect Chicago is a new initiative to expand community-based COVID-19 testing developed as a collaboration between Rush University Medical Center and Esperanza Health Centers in partnership with the Chicago Department of Public Health.

COVID-19 testing will be done at two locations and through at-home testing kits. The physical sites will also serve as a place for community residents to connect to the health care system and learn more about what resources are available at Rush and locally.

Important instructions for entering the COVID-19 testing site

To access the testing site, please use the community center entrance, which is the set of doors that face only Pulaski. Please DO NOT USE the entrance located on the corner of Pulaski and 26th Street, which is the entrance to the bank and not the testing site.


Parking for COVID-19 testing is ONLY available in the parking lot marked Customer Parking WEST. Please DO NOT PARK in the lot next to the building. YOU WILL BE TOWED if you park in the lot marked Customer Parking EAST.
