If you have insurance with a participating plan, just confirm your insurance information when you schedule your visit and you will not need to pay an upfront fee today for your visit. We will submit a claim to your insurance provider after your visit and you will be responsible for any co-pays or coinsurance that may apply, as determined by your plan. If we do not have your insurance on file, if you do not currently have insurance coverage, or if you have coverage but elect to pay for this service out-of-pocket, you will pay $99 in advance of receiving this service.

By electing to pay out-of-pocket, you understand that by making this election you waive your right to subsequently bill a health plan for the item(s) and/or service(s) you are receiving today. You acknowledge and agree that you will pay directly for any items and/or services provided to you and Rush will not bill any other third party, including any federal or state health care program, for such items and/or services.
