How it works

SmartExam is available to people 18 and older who have visited a Rush primary care physician in the past 12 months. Follow the steps below to get started:

  1. Request a SmartExam account in MyChart. (If you haven’t signed up for MyChart, free access to your online health record, you’ll need to do that first. Sign up for MyChart here.)
  2. Log in to your SmartExam account.
  3. Fill out a secure online questionnaire including 25 to 75 questions, depending on your condition. On average, the questionnaire takes 12 minutes to finish.
  4. Less than an hour later, receive an email containing a secure link to a diagnosis and treatment plan created by a Rush provider. This plan will include any needed prescriptions, which Rush will send to your pharmacy of choice to be filled.

SmartExam is not for emergencies. If you have an urgent medical need, call 911 — or visit your nearest emergency department or urgent care facility.

SmartExam hours

  • Monday through Friday, 8 a.m. to 7 p.m.
  • Saturday and Sunday, 8 a.m. to 3 p.m.

If you complete a SmartExam questionnaire outside of these hours, you will receive a response within an hour after SmartExam services at Rush begin again at 8 a.m.

Conditions treated

  • Acne (requires uploaded photo)
  • Allergies
  • Bladder infection or urinary tract infection
  • Burn or sunburn (requires uploaded photo)
  • Cold or cough
  • Ear pain
  • Flu
  • Rashes and other skin conditions (requires uploaded photo)
  • Sinus pain or pressure
  • Sore throat

If you need treatment for a condition other than these, you can make an appointment with a physician at Rush or visit one of Rush’s walk-in care locations.


Rush doesn’t bill insurance for SmartExam. Your credit, debit or health savings account (HSA) card will be charged $30 after you receive your diagnosis.

If it turns out that we need to see you before we can diagnose your condition, we’ll help you schedule an in-person appointment, and you will not be charged for SmartExam.
