Exploring Connections Between the Social Determinants of Health and Falls in Older Adults

This study’s goal is to explore how the social determinants of health influence the risk of falls among community-dwelling older adults in Cook County, Illinois, and to examine whether integrating social care interventions can enhance falls prevention strategies.

In order to participate you must meet the following criteria:

• Are at least 65 years of age.
• Have had falls or have significant fall risk.
• Are fully able to consent without major cognitive impairment.
• Reside in Cook County. 
• Speak English or Spanish.
