Individualized Care Plan

The treatment program is an individualized care plan that includes diet, exercise and/or medication, and teaches patients how to administer insulin, use an insulin pump and read a glucometer. Comprehensive diabetes education consists of:

  • Assessment
  • Developing and implementing a plan of care specific to the standards set by the American Diabetes Association
  • Goal setting
  • Evaluation
  • Nutrition Counseling

A referral from your physician is required to enter the program.  Referrals must be faxed before an appointment can be scheduled. Most insurance plans, including Medicare, provide coverage for diabetes education.
