Strengthening Handoffs to Reduce Risk

Safe, efficient surgical preparation depends on clear communication — especially during shift transitions. In the prep areas, the handoff between evening and morning charge nurses is a critical moment that sets the tone for the entire day. When that exchange lacks structure or visibility, the consequences can include workflow disruption, staffing inefficiencies and increased risk to patient safety.

Prep charge nurses identified a critical gap in this transition. Morning charge nurses often began their shift without full awareness of overnight patient changes, high acuity needs or last‑minute operating room additions. At the same time, evening charge nurses had limited insight into how morning staffing resources would be deployed. This misalignment led to reactive decision‑making, delayed patient preparation, and unnecessary operational strain.

A Frontline-Led Solution

To address this vulnerability, the prep charge nurses collaborated to design and implement a PM-to-AM Handoff Tool — a structured, reliable communication tool that captures key operational details from the evening shift. The tool includes:

  • Patient volumes across the three prep areas
  • Staffing levels, including nurses and patient care technicians
  • High-acuity patient considerations (e.g., cardiac or intermediate care) that require more intensive nursing support
  • Anticipated OR add-ons or workflow challenges

Each evening, a PM charge nurse completes the handoff sheet and distributes it to the following day’s three AM charge nurses, the nurse clinical manager and the assistant unit director.

The handoff tool has transformed the start of the day. Morning charge nurses now begin their shifts with situational awareness and a clear, actionable staffing plan. Early visibility into patient needs allows for proactive nurse allocation — especially for highacuity patients — reducing the risk of operating room delays.

In addition, the tool has facilitated improved communication and accountability across shifts while providing leadership with timely insight into staffing challenges and opportunities for support.

This initiative reflects the power of frontline leadership. By standardizing communication, improving continuity and anticipating patient needs, nurses transformed areas of vulnerability into dependable processes.

By creating a predictable and structured handoff, the prep charge nurses have advanced operational efficiency, strengthened collaboration and — most importantly — supported safer, more seamless surgical care for every patient, every day.
