We offer payment options through CareCredit. CareCredit works just like a credit card, but is exclusive for healthcare services. CareCredit is different from a regular credit card. Use it to pay for out-of-pocket expenses not covered by medical insurance, and special financing options are available that you may not be able to get with other cards.

One Medical Passport

One Medical Passport is a secure, HIPAA-compliant, web-based system that streamlines our admission process for patients, office staff and medical professionals.

Patients scheduled for procedures at our surgery center are asked to complete their medical history online using One Medical Passport well in advance of their procedure. Not only does this save patients time on the day of the procedure, but it also provides medical and office staff valuable health information about patients prior to their procedure.

Completing your medical history online using One Medical Passport is easy, and for most patients, takes less than 30 minutes. Please fill out the questionnaire accurately and completely. At any time, you may stop filling out the questionnaire and return to complete the form at a more convenient time.

What You Will Need to Fill Out the Online Form

Be sure to have the following information available before you start to fill out the online form:

  • Your health insurance information
  • The names, addresses and phone numbers of your physicians
  • A list of all medications you are taking, their dosage and frequency
  • A list of surgical procedures you have ever had
  • The name of the surgeon who will be performing your upcoming procedure

After you complete your medical history online, you will be able to print out a copy of your medical history and keep it for your records. In addition, you will be able to access your medical history online to review or update it.

Patient Financial Responsibility

Please be aware that RUSH Oak Brook Surgery Center has separate financial and billing policies and procedures from your surgeon’s office. You are most likely to receive three separate bills. One from the RUSH Oak Brook Surgery Center for facility services, one from your surgeon and one from the anesthesiologist. The surgeon and anesthesiologist charges are independent of the RUSH Oak Brook Surgery Center facility fee charge.

Please be advised that your services will be billed to your insurance plan. However, you are responsible for the payment of your in-network/out-of-network deductible, co-payments and/or co-insurance at the time of your procedure. These fees are mandated by your insurance carrier and cannot be waived.

Please be prepared to pay these fees at the time of service. We accept cash, check, and all major credit cards.

Request Medical Records

We want to make it as easy as possible for you to request copies of your medical records. In compliance with state and federal regulations, you must submit your request in writing.

How to request your records

  1. Download a form authorizing RUSH Oak Brook Surgery Center to release your health information.
  2. Fill out the information on the form, indicating the records you are requesting and the specific date of your visit.
  3. Sign the form and send it to the address below or to robsc.medrecs@scasurgery.com

Fee Schedule for Medical Record Requests

RUSH Oak Brook Surgery Center shall charge a fee for medical records which are not sent directly to another medical facility. There is no cost for records sent directly to another medical provider, hospital, clinic or doctor.

All patients requesting their records directly will be charged a flat fee of $6.50.

All other requests will be charged in alignment with the current fees as regulated by Illinois Code of Civil Procedure 735 ILCS 5/8-2001(d). Postage for records which are mailed will also be charged if applicable.

Please allow 7 to 14 days for processing.
