Fees

We share your concern about the increasing costs of medical care. Because statements and billing fees have become so expensive and in an effort to keep your medical costs down, payments are due at the time of service. Fees for surgery include the follow-up care, but do not include x-rays, casts or bandages. Fees and an estimate for service are available upon request.

Insurance and Medicare

The patient or parent/legal guardian is responsible for payment. Payment is due at the time of service. If you have insurance coverage, we will submit the necessary insurance forms to your insurance carrier. We remind you that most insurance contracts, including Medicare, involve deductibles and/or percentage allowances with the result that the entire bill is seldom covered in full. Prescription Refills

Please call 630-978-3800 to request a prescription refill. Refills are handled during regular business hours.

We will make every effort to keep our appointment schedule on time but emergencies and other unforeseen events sometimes cause unavoidable delays. In such cases, we will do our best to keep you informed.

If you are unable to keep a scheduled appointment, please contact the office as soon as possible.
