Clinical Services at Rush Rush Health and Aging
Program Description

Rush Health and Aging (RHA) offers innovative programs and services designed to measurably improve health and quality of life. From direct care coordination to health promotion and disease prevention to research and education, our mission is to promote wellness by improving access to psychosocial and medical resources for patients, those who care for them and the community.

Rush Health and Aging and Rush Geriatrics' award-winning programs rethink aging by translating innovative research into services people need to lead vital, more fulfilling lives as they age, while educating multiple disciplines to improve practice and delivery of care.

Services Provided
  • Health promotion and disease prevention — Through classes, workshops, screenings and lectures, Rush patients and people in the community benefit from evidenced-based instruction by master trainers and certified class leaders. Learn more and see upcoming events, workshops and classes. 
  • Social work services — A holistic approach to health and wellness focusing on life outside the medical exam room. Through assessment and connection to resources, social workers, as part of the patient’s interprofessional team, help address non-medical issues that may affect adherence to care plans. Support and education groups, psychotherapy and benefits counseling are also available. More information
  • Transitional care — Transitioning from hospital to home is frequently challenging. Our Enhanced Discharge Planning Program provides care coordination for at-risk seniors as they transition from hospital to home. Patients with complicated care plans may receive a follow-up phone call from a transitional care social worker after they are discharged to the community. Learn how to be an active participant in your transitional care and see family caregiver resources.

  • Professional development and affiliations — Rush Health and Aging is nationally recognized for its educational and applied research initiatives and provides training for professionals and internship opportunities for students. Geriatric Services and Older Adult Care: Rush offers older patients and their families a "one-stop shopping" experience, whether it's for a complete geriatric medical assessment, treatment for disease and disabilities (including Alzheimer's and other dementias) or even classes for seniors on how to surf the Web.

: Rush Health and Aging team members are leaders in national professional organizations that promote educational and applied research initiatives in aging. RHA promotes and supports the development of practitioners in aging through departmental internship opportunities.

Health and Aging Social Work Services: Rush Health and Aging (RHA) provides social work services to adults through the Resource Centers as well as several Rush physician practices. RHA social workers have the expertise to address non-medical and mental health needs using a culturally competent and patient-centered approach.

Rush Alzheimer's Disease Center: The Rush Alzheimer's Disease Center at Rush University Medical Center in Chicago, Illinois, is dedicated to reducing disability due to Alzheimer's disease and other age-related neurological conditions through clinical care, education, and research.
