Discharge Planning

Prior to discharge, our team works closely with the patient and family to develop a plan to address any ongoing rehabilitation needs.

We provide referrals to trusted partners across the continuum of care, including home health care and other services, based on each patient's needs. Some of our own services available to patients after discharge include the following:

Outpatient Rehabilitation

Patients can continue to receive specialized care at Rush as an outpatient, with individual services that include physical therapy, occupational therapyspeech-language pathology, audiology and/or psychology.

Prosthetics and Orthotics Clinic

A team that includes a physiatrist, licensed prosthetist and physical therapist assesses patients for devices that can help improve their function.

Wheelchair Clinic 

A multidisciplinary team evaluates patients for a variety of standard and motorized wheelchairs that offer more freedom and mobility.

Driving Rehabilitation

We can help patients regain their confidence and skills behind the wheel so they can maintain their independence.
