Outpatient Laboratory Registration
All
outpatients must be registered at Outpatient Registration prior to receiving
any laboratory service. The patient must present to the Outpatient Registration
area a completed laboratory requisition from the patient's physician. The use of the laboratory requisition is
strongly encouraged to insure completeness and correctness of submitted
information. The following information
is required on the laboratory requisition:
- patient's full name, sex, and date of birth
- medical record number (if known)
- physician's full name
- physician's group practice (if applicable)
- tests requested
- ICD-9 Code or diagnosis
Billing
Billing for
laboratory tests is done at the time of outpatient registration. In addition to
insurance information, the patient should bring identification (eg, driver's
license, photo ID). Payment and billing varies dependent upon the patient's
medical coverage.
Blue Cross/Blue Shield of Illinois
The patient
must present the Blue Cross/Blue Shield card as proof of insurance. BC/BS will
be billed. Patients carrying out-of-state BC/BS may not qualify for coverage;
cash payment is necessary.
Other Commercial Insurance
The patient
must present a completed insurance claim form for direct billing of the
insurance company. Patients without claim forms must pay by cash or credit
card.
Medicare and Public Aid
The patient must present a current Medicare/Medicaid (public aid card). Medicare/Medicaid is billed directly.
RUSH-PRUDENTIAL Health Plan and RPSLMC Contracted Providers
The patient
must present an enrollment card and/or authorization form. Certain HMOs and
other providers, which are not contracted with RPSLMC, must pay cash. Contact
ext. 25841 for information on contracted providers.
Uninsured/Noncontracted Providers
Cash payment
is required. Cash, personal check, MasterCard, Visa, and American Express are
accepted.
Contracted Services
Many patients' insurance requires a specific laboratory
perform the testing. Patients must
present a requisition from the required laboratory that has been completed by their physician's
office. The specimen(s) will be collected and sent to the specified laboratory.
The Outpatient Laboratory provides this service for specimens that must be referred to Quest Diagnostics, Prometheus Labratory, Athena Diagnostics, NMR Labs, and Lab Corp. In some instances the patient or their insurance company may receive a bill from these laboratories for the testing performed.
