CHART: Congestive Heart Failure Adherence Redesign Trial

CHART is a study aiming to reduce repeated hospitalization in low-income congestive heart failure patients and improve each patient's ability to maintain health-related recommendations. This multi-level intervention attempts to reengineer the relationship between the doctor and the patient.

You may be eligible for this study if you:

• Are male/female 18 years or older
• Have been diagnosed with heart failure
• Have an annual income of $30,000 or less
