international appointment request form.

Please complete all four portions of the form for it to be submitted to our staff.

Step 2. If additional records or information are required or requested, please send them via mail or fax.

Please note: All records must be in English. Please use a medical transcription service to transcribe your medical records.

Step 3. Once we receive your medical records, our staff will work with you to set up your appointments.

Please inform us if any family members will be traveling with you and if they will also require medical services, so we can plan your stay better.
