Free Personal Health Assessment

My Rush Health Advisor can be your partner in health. Doctors at Rush University Medical Center in Chicago, Illinois, have developed this tool using guidelines published by the U.S. Preventive Services Task Force, an independent panel of experts in primary care and prevention. Physicians at Rush have used these recommendations — along with their own experience caring for patients — to help you determine your health risks and guide you to lifestyle changes, diagnostic tests and immunizations that may help you lead a healthier life.

My Rush Health Advisor includes sections on the following areas:

  • Women’s and men’s specific health concerns
  • Cancer prevention
  • Diet and nutrition
  • Heart health
  • Depression
  • Hearing and vision
  • Home safety

You do not have to take the assessment in one session — you can come back to it at any time. Nor do you have to complete every section. You can jump to those that concern you most using the selection menu on the left-hand side of the screen.

How can this tool help me?
The assessment will give you information about lifestyle changes that will help reduce your risk of developing certain diseases. It will also recommend screening tests and immunizations that have proven benefits. The assessment will help you identify preventable risk factors — such as smoking — that you can change to improve your health.

How can I be assured of confidentiality?
As you move through the program, we will ask you questions about your age, gender, family history, lifestyle, diet and other factors. We will use this information to make recommendations for healthy living. However, we will not capture any personally identifiable information during the assessment, so your privacy is ensured.

Do I need a special computer to take this assessment?
You do not need a special computer, but you will need a computer equipped with certain capabilities. We recommend a computer with sound and with a broadband connection (cable or DSL). You will also need to have a Flash player installed to use the assessment. Most computers are equipped with a Flash player — if yours is not, go to www.adobe.com/products/flashplayer/ to download the free software.

What will the assessment tell me?
Based on your answers, we’ll offer you specific recommendations for healthier living. These may include recommended tests and immunizations, and topics for discussion with your physician. You can print these recommendations as a report at the end of the assessment. We encourage you to use this report as a guide for developing a health action plan with your physician.

How much does this cost?
My Rush Health Advisor is a free service of Rush University Medical Center.

I don’t have a personal physician. Can I make an appointment with a primary care physician at Rush?
When you are finished with the assessment, your report will include a link to a form for requesting an appointment with a physician or for any of the services we recommend. The report also includes a link to our online physician database. You can always call (888) 352-RUSH, toll free, to make an appointment with any physician at Rush.

How do I get started?
Go to myrushhealthadvisor.com to begin your personal health assessment. It’s easy, free and confidential.

To take the assessment, you’ll need the following:

  • A computer equipped with a broadband connection, such as cable or DSL.
  • A few minutes — the assessment takes 20 to 30 minutes to complete in its entirety, but you don’t need to take it all at once. You can start and come back to the assessment at any time. And you don’t have to take the whole assessment — you can focus on the health topics that concern you most.

Based on your answers, we’ll offer you specific recommendations for getting and staying healthy. You can print these recommendations as a report at the end of the assessment.
