We make every effort to accommodate your schedule, but please be aware that all travel/immunization visits are by appointment only.
Travelers should allow plenty of time to complete this process — ideally immunizations should be administered by at least 30 days prior to travel, and some vaccination series require initiation six to eight weeks prior to travel.
If you would like to use any of our services, you will need to fill out a brief travel health history form. A separate form is required for each traveler. When we receive your completed form, we will initiate preparation for your visit. Please be sure the forms are complete and signed before returning them — incomplete forms may delay processing.
