A copy of your medical records

All information on patients is stored in a confidential manner. Copies of medical records can be obtained by request, as specified by the Health Insurance Portability and Accountability Act (HIPAA) regulations. We cannot release the original copy, however. If you are transferring to the Rush HTC from another provider, it is your responsibility to have previous medical records sent to Rush HTC before your visit or your child’s first visit. We would be pleased to provide you with a medical record release form if you wish. In addition, to maximize the value of your upcoming comprehensive clinic visit, we suggest:

• Bring a pair of shorts for yourself or your child to wear so the doctor and physical therapist can fully examine the joint.
• Arrive on time. Your day will be busy; to have all evaluations performed you must stay on schedule. If this is your first visit, please come 15 minutes early.
• Bring factor infusion logs and shipment records from your home care company.
• Make a list of all of your questions and concerns before arriving in the office.
• Make sure you bring your referral from your primary doctor to the visit, if needed.
