Tribute and memorial gifts offer a thoughtful way to remember a loved one or friend, to recognize a doctor or nurse at Rush, or to celebrate a special milestone such as a birthday or anniversary. A gift of any type or size is welcome.

If you choose to establish a tribute or memorial:

• We will provide special tribute envelopes that your friends and family can use to submit gifts to Rush. The form includes a space for the donor to indicate where the gift should be directed.
• When we receive a gift designated for your tribute, we immediately thank the donor. We will also promptly let you know the names and addresses of the generous people who have contributed.

If you would like to make a tribute or memorial gift:

• You may donate by simply mailing a gift to the address below or give online.
• When mailing your gift, please include the name of the person (or people) you wish to honor or memorialize and the contact information for anyone who should be notified of your generosity.
