Tax Information for Incoming House Officers

All house officers must complete two W-4 forms (one federal and one state). You may complete the bottom of the forms on the computer for legibility and print out to sign. Complete the bottom part only of each form and return them to the GME office as soon as possible. These forms cannot be faxed as your original signature is required.

NOTE: If you do not have an Illinois address, you may complete the form, BUT DO NOT FILL IN YOUR OUT-OF-STATE ADDRESS. Leave the address blank and inform GME as soon as you have a local address so your form can be processed.

You do NOT need to complete the worksheet portion. If you have questions about the best number of allowances to declare, refer to the W-4 guidelines below.
