Applicants will use the Rush Fellowship Application Form. Completed applications with three letters of recommendation from professional sources are required, including a letter from the director of the applicant's most recent training program.

Please note that an absolute requirement for training at Rush University Medical Center is a current, permanent (#036) Illinois license by July 1st of the fellowship start year. Graduates of foreign medical schools are candidates for the Illinois license with ECFMG certification or completion of USMLE Steps 1 , 2 and 3 along with a minimum of two years in an ACGME, ACCGME or AOA residency training program. Questions regarding licensure may be directed to Graduate Medical Education at (312) 942-5495 or may be answered on the Illinois Department of Professional Regulations website at www.dpr.state.il.us which indicates the requirements for permanent medical licensure in Illinois.
