Application Information

The Rush Primary Care Sports Medicine Fellowship Program accepts one fellow per year.

We are currently accepting applications for the 2009-10 academic year. To apply to our program, please complete the Universal Application available on the AMSSM website. We require three letters of recommendation, curriculum vitae and personal statement as well any other documents requested on the application form.

The deadline for applications is October 31, 2008.
