Dear Incoming House Officers:
There is so much information and so many forms, BUT getting them all done ahead of time will free you from these concerns before you start at Rush! You legally have to complete these forms within ten days of your start date or you risk being ineligible to enroll until the next open enrollment period (see specific dates in red under each benefit narrative). The only exceptions are qualifying events such as marriage/divorce, birth/adoption, etc., but we are asking you to complete them NOW so we can process them ASAP. That way, your membership can be processed and ready.
Before you start this process, please print out the GME insurance plan checklist
and indicate exactly what insurances you are applying for. Send this sheet with the appropriate application forms (and the crucial copies of marriage certificates and/or birth certificates for any/all dependents you are covering), no later than May 30th.
IMPORTANT: Please know that the internal links on the applications (in Adobe PDF format) can only be opened and used on PCs. The forms may be completed on-line, but will then need to be printed out for signature and submission. These links and forms do not work on MACs (which opens PDF files in the default Preview software instead of Adobe Acrobat Reader). We apologize for this inconvenience. There you will find an overview of the many perks and benefits that are offered here at Rush. You should click on the red link for “specific information and application forms” to review all the benefit packages offered including several optional ones. Please click on each “information” link to learn more about each specific benefit. Then click on the “application form” link to apply. Most forms may be completed on your computer, but then need to be printed out and signed with original signatures. If you have questions, there is contact information for each benefit listed. Please do not hesitate to call them to get your questions answered and help you make educated decisions.
