Application Process
A potential applicant to the Midwest Hospice and Palliative Medicine Integrated Fellowship program must be a licensed physician in the United States.* In addition, any resident considered for a fellowship must:
- Complete an ACGME-approved residency program prior to application
- Be board certified or eligible by the American Board of Internal Medicine or the American Board of Family Medicine. (Other specialties would be considered.)
- Complete the Application for Fellowship in Palliative Medicine. All information requested on the form must be entered in full (e.g., reference to curriculum vitae is not acceptable). Please include telephone and pager numbers where you can be reached.
- Provide a personal statement.
- Provide three letters of recommendation.
- Describe a plan of service in the practice of palliative medicine that enhances access to supportive care for patients and families as well as lay and professional communities.
- Interview with the program director, members of the interdisciplinary team and faculty representatives.
- Demonstrate commitment to the field of palliative medicine and the desire to pursue advanced training in this field.
The Midwest Palliative & Hospice CareCenter Fellowship Committee reviews completed applications and awards the fellowship position(s) based on merit. (The review committee includes the program director, section chief and members of the interdisciplinary team.)
* Graduates of international medical schools also may be eligible. Those applicants are required to submit either a permanent certificate (valid indefinitely) from the Educational Commission for Foreign Medical Graduates or comparable credentials from the Medical Council of Canada.
