Urethral Strictures and Disruptions

In bladder drained pancreas transplants, males may have a difficulty with irritation of their transmembranous urethra. A small percentage of males will develop disruption of their urethra with subsequent urine extravasation. These can be managed with early detection and non-operative treatments. The patient will complain of a fullness and pain in the perineum. Cystogram will document the urethral disruption and extravasation. Commonly there will be a distal urethral stricture identified which has contributed to this disruption. Foley cathether drainage with stricture dilatation for two weeks has been carried out with success, thus avoiding operative reconstruction. Again, enteric drainage of the pancreas avoids these problems.
